Some believe it wouldn't add much content since all 2nd gen pokemon are in game. I personally think that new region and gyms are appealing and would be a nice addition. Maybe they will add Johto after making content they are working on right now? Who knows :)

Pokemon Crystal is a generation 2 game, its visuals would be drastically different than that of Heartgold / Soulsilver and for that reason in order to keep visual graphics in line with current regions if Johto were to be added to the game I suspect that Heartgold / Soulsilver would be used as the visual base.

That is not to say that some minor elements from Pokemon Crystal would never make it into the game, PokeMMO tends to take many versions of things into account when considering encounter rates for regions (e.g. we have Eevee in Hoenn due to its appearance there in Omega Ruby / Alpha Sapphire despite it being based on gen 3's Emerald Version).
As a result, it's possible that some minor aspects of Crystal (such as Elite 4 teams or wild encounter rates on some routes) may be present, but the game would be, for the vast majority, based more on HG/SS than Crystal.

Though, nothing is confirmed in terms of Johto as a region so far, so this is largely just speculation based on precedents set by previous regions. remember to take them with a pinch of salt.

Haben wir hier Evoli in Hoenn, aufgrund seines Auftretens in Omega Rubin / Alpha Saphir, obwohl die originale generation 3 Smaragd Version als Basis verwendet wurde).

Daher kann es sein, dass einige Kernaspekte aus Kristall (wie Top 4 Teams oder Erscheinungsraten) vorkommen könnten, aber die Mehrheit wird wohl auf HG/SS basieren, anstelle von Kristall.

 

Dennoch ist bisher noch nichts bestätigt was mit Johto als Region zu tun hat, weshalb alles nur Spekulation ist, die auf Grundlagen der bisher genutzten Regionen basiert. Denk dran, dass nicht alles stimmen oder so passieren muss wie es hier gesagt wird.
